DEVICE PICK UP INFORMATION
Before you can pick-up a device from school, we need to have information so that we can prepare your device and make the pick-up process easier and more convenient. Once you choose a time slot, please complete the following form to request a device from the school. Please fill out the following information with your STUDENTS’ information:

This is the information that will be collected:
- Student Name (First, Last)
- Parent or Student E-mail
- Parent Cell Phone Number
- Student Lunch Number
- How many devices do you have in your household?
- How many students do you have in your household?
- Do you have internet access?
Once you complete the form with the requested information, please submit your request. Write down your appointment time. You will not receive a reminder. Pick Up only – Delivery is not available. (1 device per 2 students recommended.)
Please come to the AM Car loop (first loop on the right when you enter the school) at your designated time. It is important that you come on time so we can stay within the guidelines and restrictions from the CDC. Please stay in your car and a staff member will assist you. Thank you!
